Backfilled PR for already-completed task fa0834ec1ba7 (predates the reviewer pipeline).

Define the avatar attachment spec: how body parts attach, what slots exist (head, body, legs, accessories), and the glTF/GLB format requirements. Write spec in docs/architecture/avatar.md.
